Welcome to LHS Virtual-Care, a service provided by Livingdream Health Services LLC. We are dedicated to offering digital healthcare solutions, telemedicine consultations, patient engagement tools, and various medical resources to facilitate a seamless experience for patients and healthcare providers. The terms, conditions, and disclaimers outlined in this document ("Terms") govern your access to and use of the LHS Virtual-Care platform, including but not limited to: patient portals, telehealth sessions, scheduling tools, messaging functions, electronic health records review, prescription management, and related services offered now or in the future.
These Terms apply to all users, including patients, authorized patient representatives, medical professionals interacting through the system (where applicable), and any other parties accessing the services. By using our platform, you acknowledge that you have read, understood, and agree to be bound by these Terms. If you do not agree, please discontinue use immediately.
Our mission is to enhance patient care, improve accessibility to healthcare services, and ensure a secure, confidential environment that respects patient rights. We continuously strive to implement rigorous security standards, maintain the accuracy of medical information, and foster a supportive environment. However, these services are not intended to replace in-person clinical evaluations when such care is necessary.
In addition, these Terms incorporate any other agreements you have entered into with LHS Virtual-Care and Livingdream Health Services LLC, including but not limited to consent forms, privacy notices, and practice agreements. To fully understand your rights and obligations, please review all applicable documents.
2. Emergency Disclaimer (Call 9-1-1)
DO NOT USE LHS VIRTUAL-CARE FOR EMERGENCIES. If you think you may be experiencing a medical or psychiatric emergency, call 9-1-1 or go to your nearest emergency department immediately. LHS Virtual-Care services are not designed for urgent or emergent healthcare needs, such as severe chest pain, difficulty breathing, signs of stroke, severe mental distress with immediate risk of harm, severe bleeding, or other critical conditions requiring prompt in-person intervention.
Our platform is intended for non-emergency medical care, follow-ups, prescription management, routine consultations, and other forms of care that do not necessitate immediate clinical intervention. By using LHS Virtual-Care, you agree not to rely on the platform for situations that require immediate emergency medical treatment.
3. Scope of LHS Virtual-Care Services
LHS Virtual-Care is a technology platform that facilitates remote healthcare services through secure video conferencing, messaging, and digital records sharing. The scope may include, but is not limited to:
- Telehealth consultations: General medical advice, consultation, and follow-up appointments using video or phone-based services.
- Access to health records: Viewing laboratory results, imaging studies, progress notes, and prescription histories.
- Prescription management: Requesting prescription refills or updates where legally and clinically appropriate.
- Scheduling and appointments: Booking, rescheduling, or canceling virtual or in-person visits with healthcare providers.
- Secure messaging: Non-urgent, routine communication with healthcare teams for clarifications, follow-ups, and health education.
- Preventive care reminders: Notifications about recommended screenings, immunizations, or routine check-ups.
While we strive to expand and improve our services, LHS Virtual-Care does not replace all aspects of in-person care. There may be limitations to diagnoses, interventions, and treatments that can be provided virtually. Certain conditions, complexities, or situations may require referral to an in-person physician or specialist.
LHS Virtual-Care does not create a patient-physician relationship in all contexts. That relationship is established only when a licensed medical professional reviews your information and engages with you in a clinical capacity. Please note that not all services will be available in all geographic areas due to regulatory, licensing, or other legal considerations.
4. User Responsibilities & Representations
By using LHS Virtual-Care, you represent and warrant that you:
- Are at least 18 years old or, if accessing for a minor, are a parent or legal guardian with appropriate authority.
- Will provide accurate, current, and complete information during registration and while using the platform.
- Will update any personal, contact, or medical information promptly to ensure accuracy.
- Understand that certain services depend on the accuracy of the information you provide, and that inaccurate or incomplete information may affect the quality of care or recommendations.
- Will not use the platform for unlawful purposes or in violation of any applicable local, state, federal, or international law, including but not limited to healthcare regulations and privacy laws.
- Will not impersonate another person, misrepresent your identity, or falsely claim authority to act on behalf of another individual.
- Will not misuse the communication tools by sending spam, harassing messages, or offensive content.
You are responsible for maintaining the confidentiality of your account credentials, including usernames and passwords. Any unauthorized use of your account should be reported immediately to: support@lhsme.com.
5. Privacy & Security Measures
LHS Virtual-Care is committed to protecting your privacy and securing your personal health information in compliance with applicable laws, including the Health Insurance Portability and Accountability Act (HIPAA).
Data Collection and Use
We collect and use personal information necessary to provide healthcare services, including:
- Personal identification details (name, date of birth, contact information)
- Health and medical information (medical history, symptoms, treatments)
- Insurance and payment information
- Technical data (IP address, device information, usage patterns)
Data Security
We implement comprehensive security measures to protect your information, including:
- End-to-end encryption for all data transmissions
- Secure storage with access controls and audit trails
- Regular security assessments and vulnerability testing
- Employee training on privacy and security protocols
Data Sharing

- Healthcare providers involved in your care

Data Accessed
LHS Virtual-Care only accesses Google user data necessary to provide its core functionality. Specifically, we access:
- Google Calendar events and metadata for scheduling, updating, and managing patient appointments.
- User profile information (name and email address) to identify accounts and associate appointments.
Data Usage
The data we access is used solely to:
- Allow patients to book, view, and manage their appointments through LHS Virtual-Care.
- Enable healthcare providers to view and update their schedules.
- Send appointment reminders and confirmations via email if the user opts in.
Data Sharing
We do not sell or share Google user data with third parties for their own marketing or commercial purposes. Data is shared only as follows:
- With authorized healthcare staff within LHS Virtual-Care as required for scheduling and patient care.
- With third-party service providers (e.g., secure hosting, email delivery services) who are bound by contracts requiring confidentiality and secure handling of personal data.
Data Storage & Protection
All Google user data is stored securely on our HIPAA-compliant servers. We are committed to protecting your data and implement robust security measures, including:
- Industry-standard encryption (SSL/TLS) for data in transit.
- Encrypted storage for sensitive data.
- Firewalls, intrusion detection, and regular security audits.
- Strict access controls limiting data access to authorized personnel only.
Data Retention & Deletion
User data is retained only as long as necessary to provide services or as required by law. Users can request deletion of their data in two ways:
- Direct Request: Contact our Privacy Officer at privacy@lhsme.com. We will process verified requests and remove the data from our systems within 30 business days, providing confirmation upon completion.
- Revoke Access: Users can instantly revoke our application's access to their Google data at any time via their Google Account permissions settings (at https://myaccount.google.com/permissions).
6. Medical Content Disclaimer
The information provided on LHS Virtual-Care is for educational and informational purposes. It is not a substitute for professional medical advice, diagnosis, or treatment provided by a qualified healthcare practitioner who is familiar with your individual medical history, condition, and needs.
You should not rely solely on the platform's content or electronic communication with clinicians for critical medical decisions. Always seek the advice of your physician or another qualified health provider with any questions you may have regarding a medical condition. Do not delay seeking professional medical advice because of something you read or received through LHS Virtual-Care.
We strive for accuracy, but medical knowledge evolves rapidly. We cannot guarantee that all content, guidance, or information is always up-to-date, comprehensive, or appropriate for every patient. It is your responsibility to confirm the accuracy and relevance of any information with a qualified healthcare professional.
7. Mental Health & Psychiatric Emergencies
If you are experiencing suicidal thoughts, severe depression, hallucinations, delusions, or any mental health crisis that may place you or others at imminent risk, call 9-1-1 or visit the nearest emergency department. LHS Virtual-Care is not a suicide prevention, crisis intervention, or emergency psychiatric care service.
The mental and behavioral health services provided through LHS Virtual-Care are designed for routine therapy sessions, follow-up consultations, medication management (where appropriate), and ongoing care discussions with licensed mental health professionals. While these services can be highly beneficial, they are not a substitute for in-person evaluation and acute intervention when immediate safety or urgent care is required.
If you or someone you know is in crisis or needs emotional support, the following resources may also be helpful (in addition to calling 9-1-1):
- National Suicide & Crisis Lifeline (Call or Text 988 in the U.S.)
- Your local mental health helpline or emergency psychiatric service
- Seeking immediate in-person evaluation at an emergency department or urgent care facility
8. Scheduling, Booking & Cancellation Policy
Appointment Scheduling
LHS Virtual-Care provides tools for scheduling virtual consultations, follow-ups, and other appointments through our platform. While we strive to maintain punctuality and availability, appointment times are subject to change due to provider availability, technology issues, and other unforeseen factors.

To schedule or modify appointments, you may use our online scheduling system or contact us at booking@lhsme.com. We recommend confirming your appointment at least 24 hours in advance and logging in a few minutes early to ensure a stable internet connection and functional audio/video setup.
Cancellation & Rescheduling Policy
We understand that sometimes you need to change your appointment. To accommodate all patients and ensure efficient use of provider time, we have implemented the following cancellation policy:

Cancellations made on the day of appointment will be charged 50% of the service fee. No-shows (missing an appointment without prior cancellation) will be charged the full service fee.
To cancel or reschedule an appointment, please use our online portal or contact booking@lhsme.com at least 24 hours in advance to avoid fees. Emergency situations will be considered on a case-by-case basis with appropriate documentation.
Insurance and Payment Information
Please confirm your insurance coverage and copay requirements with your insurance company prior to your appointment. While we strive to verify benefits, ultimate responsibility for understanding your coverage rests with you.
Any cancellation or no-show fees are the patient's responsibility and may not be covered by insurance. These fees must be paid before scheduling future appointments.
Late Arrival Policy
If you arrive more than 10 minutes late for your scheduled appointment, we reserve the right to:
- Shorten your appointment time while charging the full fee
- Reschedule your appointment (subject to cancellation fees)
- Treat the appointment as a no-show if you arrive more than 15 minutes late
9. Electronic Communications & Response Times
LHS Virtual-Care allows for secure messaging between you and our healthcare team. While we endeavor to respond to messages promptly, please allow up to two (2) business days for a reply. For time-sensitive issues, contact our office by phone or schedule a prompt follow-up visit. Do not rely on the messaging system for emergencies, urgent care needs, or immediate responses.
Electronic communications may be retained as part of your medical record, as allowed by law. Such records may include inquiries, provider responses, attachments, and other content shared through the platform. You agree to keep your messages respectful, accurate, and relevant to your care.

10. Data Accuracy & Updates
The reliability of any recommendations, follow-ups, or medical advice provided through LHS Virtual-Care depends on the accuracy and completeness of the information you supply. You are responsible for updating your profile with any changes in address, phone number, insurance details, medical history, or current medications.
Failure to provide accurate information could result in misdiagnosis, inappropriate treatment recommendations, or delays in care. We are not liable for any harm that results from incomplete, outdated, or inaccurate information you provide.
11. Third-Party Links & External Resources
The LHS Virtual-Care platform may contain links to external websites, resources, articles, or tools for informational purposes. These external links are provided as a convenience and do not constitute endorsements. We do not control the content, accuracy, or security of external sites.
If you choose to access third-party links, you do so at your own risk. We recommend verifying the credibility and privacy practices of any external resource before sharing personal information or relying on its content. Always seek guidance from a qualified professional before acting on health-related information found elsewhere.
12. Proxy Access & Minors
If you are the parent or legal guardian of a minor patient, or a legally authorized representative (e.g., healthcare power of attorney), you may request "proxy access" to manage or view health information on behalf of that individual. Proxy access requests must comply with applicable laws and regulations, and you may be asked to provide documentation of your authority.
Certain privacy laws and regulations may limit the type and amount of information accessible for minors, especially adolescents. We reserve the right to deny or revoke proxy access for any reason, including changes in legal custody, guardianship, or upon a minor patient reaching an age where they are granted confidentiality rights by law.
13. Termination & Revocation of Access
We may terminate or revoke your access to LHS Virtual-Care at any time, with or without cause. Reasons for termination may include but are not limited to:
- Violation of these Terms, any applicable laws, or privacy regulations
- Evidence of fraud, misuse, or tampering with the platform
- Provider decisions based on clinical or administrative grounds
- Request by you to discontinue your use of the platform
In the event of termination, you will no longer have access to your account or any data stored within the platform, subject to applicable laws governing record retention. If you believe access has been terminated in error, please contact support@lhsme.com.
14. Warranties & Liability Limitations
LHS Virtual-Care services are provided on an "AS IS" and "AS AVAILABLE" basis. We make no warranties, express or implied, regarding the platform's fitness for a particular purpose, merchantability, or non-infringement. We do not guarantee that the platform will be error-free, secure, or available uninterrupted.
To the maximum extent allowed by law, neither LHS Virtual-Care, Livingdream Health Services LLC, nor any of their respective owners, employees, contractors, or affiliates shall be liable for any direct, indirect, incidental, special, consequential, or punitive damages arising from your use or inability to use the platform. This includes damages for loss of data, loss of income, business interruption, personal injury, or other harm.
15. Indemnification
You agree to indemnify, defend, and hold harmless LHS Virtual-Care, Livingdream Health Services LLC, and their respective affiliates, employees, agents, and providers from and against any claims, liabilities, damages, judgments, awards, costs, and expenses (including reasonable attorneys' fees) arising out of or related to:
- Your use or misuse of the platform
- Your violation of these Terms, any applicable law, or regulation
- Any information you provide, including inaccuracies or misrepresentations
Your obligation to indemnify remains even after you cease using LHS Virtual-Care.
16. Amendments & Updates to Terms
We reserve the right to revise, amend, or update these Terms at any time, for any reason, without prior notice. Any changes will be posted to the platform, and the "Last Updated" date at the top of these Terms will be updated accordingly. Your continued use of the platform following such updates signifies your acceptance of the revised Terms.
We encourage you to review these Terms regularly to stay informed of any modifications. If you do not agree to any changes, you should discontinue use of LHS Virtual-Care immediately.
17. Governing Law & Jurisdiction
These Terms are governed by the laws of the state where Livingdream Health Services LLC maintains its principal place of business, without regard to its conflicts of law principles. Any disputes arising from or related to these Terms or your use of LHS Virtual-Care shall be resolved in the state or federal courts of that jurisdiction, and you hereby consent to the personal jurisdiction of those courts.

19. Entire Agreement
These Terms, along with any supplemental agreements, policies, notices, consent forms, and other documents referenced herein, constitute the entire agreement between you and LHS Virtual-Care and Livingdream Health Services LLC concerning the platform and its use. They supersede any prior or concurrent understandings, representations, or agreements, whether written or oral.
If any provision of these Terms is found to be unenforceable by a court of competent jurisdiction, the remaining provisions shall remain in full force and effect.
Your use of LHS Virtual-Care indicates that you have read, understood, and agree to abide by these Terms, including all disclaimers, limitations, and obligations stated herein.
